  2. Very much heat related issue. BUT, if it were as simple as clearance, it would show in all four mostly on the middle pair, bcz they would be the hottest of the row. As you said, it idled fine with the three after one suddenly let go at the top of 5th. This is fuel related. The fuel system stopped feeding that one cylinder right before it went. Injector got clogged/noised out, or rail is inefficient/inconsistent, fuel filter got clogged so there's not enough flow in the system, the system itself is too small to feed the injectors all the way. OR the other option; back pressure creeping heat in the header. The main damage in the head was on the exhaust side. If you simply slap together another long block with wider gaps and leave the rest as it sits, this will happen again. The built bottom end was perfectly fine. It was the rest of the engine that caused the issue, bcz it happened in only one cylinder and most definitely from excess heat.
